You understand that if you receive a telephonic physician consultation with Lifestyle Medicine of New Jersey, such a consultation supplements face-to-face physician care and, therefore, Lifestyle Medicine of New Jersey healthcare providers require a copy of all pertinent medical records and a personal physical examination by your primary care provider physician before they will initiate your therapy. You understand that it is your responsibility to comply with Lifestyle Medicine of New Jersey's protocols that include the documentation of an annual physical examination along with all necessary laboratory tests to ensure that you have no underlying medical conditions that might make any therapies, including prescription drugs, inappropriate for your condition. Additionally, you agree to truthfully provide a complete and accurate medical history. You agree to allow Lifestyle Medicine of New Jersey physicians and medical staff to review all the information and requests you have provided online, in writing, or verbally to accurately evaluate you as a potential patient, and in the process of establishing/maintaining your records.
You understand it is your responsibility to immediately notify any other physician whose present care you are also under about your care from Lifestyle Medicine of New Jersey, including any prescriptions, so that they, too, may advise you concerning any potential conflicts in care and so that all your healthcare providers may consult regarding your care. If you receive the care of a new healthcare provider in the future, you agree to immediately notify that healthcare provider, as well. You understand and agree that the doctors, nurses, pharmacists, and/or other healthcare specialists employed by or affiliated with Lifestyle Medicine of New Jersey may contact you and/or your primary care provider in the event that there is a question about your medical condition or medical history.
You also attest that your medical complaint is a legitimate and documentable medical complaint and may be defined as an “emergency medical situation. ” An “emergency medical situation” may be defined as “a condition of emergency in which immediate medical care, hospitalization, or both is required by a person for the preservation of health. ” This definition may be modified in meaning to constitute the definition of a “temporary physician-patient relationship” in the state in which you reside and/or the physician resides, is licensed, and/or practices medicine.
You understand that if treatment is prescribed, it is possible that you may suffer potential adverse effects (side effects) and that no physician, nurse, pharmacist, or medical staff personnel can guarantee clinical benefit from prescribed treatments. You hereby release Lifestyle Medicine of New Jersey and all of its personnel and medical staff, including affiliated physicians and affiliated pharmacists, from any and all liability whatsoever associated with any adverse effects you may suffer from your use of your prescribed treatments. You also fully understand that if you have failed in any way to furnish Lifestyle Medicine of New Jersey with your complete and accurate medical history, or become aware of any changes in the future of which you have not notified Lifestyle Medicine of New Jersey, you cannot hold Lifestyle Medicine of New Jersey responsible for any adverse effects you may suffer.
